Hoodies Order Form

For fillable show bill and class cards:

• E-mail rntwade1@gmail.com to be included on the distribution list.  This will only need to be done one time. 
• Fillable show bill and class cards MUST be sent to rntwade1@gmail.com no later than midnight Thursday night prior to show date. 
• One horse/one rider per showbill ONLY.  Please proof read and make sure forms are correct. 
• If you do not have a back number; leave it blank.  A number will be assigned to you. 
• Coggins can be scanned and emailed to the same address. 
• Fillable forms and coggins MUST be submitted again for each show. 

• Fillable show bill and class cards MUST be emailed to me no later than Thursday Midnight prior to show.....Not responsible if not received by this time. 
  One horse only per show bill.   All information must be included correctly on forms and PLEASE PROOFREAD! Contact Terry Wade if you have any questions rntwade1@gmail.com
